COVID-19 Vaccine Online Order Form Ontario’s COVID-19 vaccine program has transitioned to an annual program in alignment with expert recommendations from the National Advisory Committee on Immunization (NACI)- Guidance on the use of COVID-19 vaccines for 2025 to summer 2026. Eligibility Criteria: All individuals ages 6 months or older who live, work or attend school in Ontario who have completed or not completed primary series. Please be reminded that the number of doses that you receive in your order may be adjusted at the discretion of the public health unit in accordance with vaccine supply. As a result, it is recommended that providers do not book appointments prior to receiving COVID-19 vaccine. You will be notified by the WECHU when your order is ready. For vaccine pick up, coolers must be pre-chilled between 2-8°C and all transport equipment must be present. If you plan to have your order delivered by Medex; please contact Medex at 519-735-7273 to arrange your delivery. Before receiving and administering any COVID-19 vaccines, review the Ministry of Health COVID-19 Vaccine Program website for COVID-19 vaccine administration, storage, and handling guidelines to ensure you have the most up to date information. COVID-19 vaccine inventory and administration are documented in COVaxON. To be able to order and administer COVID-19 vaccine you must have an active COVaxON account. To maintain an active COVaxON account, users must log in at least once every 3 weeks.
