If you suspect a privacy violation, or that Personal Information (PI) or Personal Health Information (PHI) has been improperly collected, used, or disclosed by the Windsor-Essex County Health Unit, you have the right to file a complaint.
Suspected or confirmed privacy incidents can be reported using the following webform. The submission will be sent directly to the Privacy Officer.
It is your choice if you would like to remain anonymous or provide your name, as it is not a required form field. Only the Privacy Officer will be able to view your submission, and it will remain confidential unless you consent otherwise. By providing your name, it will allow the Privacy Officer to work with you to seek clarification and investigate any incidents further.
If you are not satisfied with the response, and you wish to report it as a whistleblower, you can file a complaint with the Information and Privacy Commissioner of Ontario (IPC).
