School and Childcare Centre Absenteeism Reporting Form

When an administrator has determined their school has ≥20% absenteeism (children and staff), or if there is a concern of high absenteeism in a cluster/classroom, please complete the reporting form below. Once received, the Windsor-Essex County Health Unit (WECHU) will contact you to provide further direction.
