All formal immunization record or immunization ID access or correction requests, or PIN reset requests, must be received by the Windsor-Essex County Health Unit (WECHU) using this submission page. All other forms of request will not be processed.
An access/correction request for information will be processed in accordance with the limits set out in the Personal Health Information Protection Act (PHIPA), and the related regulations. The time limit to respond to your inquiry will begin from the date the request and supporting documents are received.
