The Windsor-Essex County Health Unit's Disease of Public Health Significance Reporting Form

Please use this form to notify the Windsor-Essex County Health Unit (WECHU) of any Diseases of Public Health Significance (DoPHS). The Health Protection and Promotion Act 1990 (HPPA), R.S.O., 1990, and Ontario Reg. 135/18, outlines the requirements for health care providers and institutions to report any suspect or confirmed Diseases of Public Health Significance (DoPHS) to the Medical Officer of Health.
